This Order Change Request are for customers who have already placed an order for product and made a mistake on the address, phone, zip, Email, etc. and want to us to make a change before it ships.

This request form is given immediate attention by our order department and if the order has not already shipped, we will make the change. We realize this is an urgent matter, please do not call, as this Request Form will take priority.

Please include your Customer/Shopper ID number. If you do not know it, please give us the full name of the person and/or company name on the order.

Please be very specific in the changes you wish to have done.
